Cloverport Independent School District

Sports Physicals will be given on September 11th during the school day.  Students will need to pick up the sports physical form in the office and Parents will need to fill out the form and sign it. The student will need to have the completed and signed form with them on September 11th so the physician can complete the physical.  If you have any questions please contact Scott Adcock at Cloverport Independent School.  No physical will be provided if the form is not completely filled out and signed.
